New patients should follow the instructions below for creating an account. When you log in, you will find yourself in a directory screen with options for Inbox, Appointments, and Settings. Please check your Inbox for messages. Through the Inbox, I will send an Intake/Authorization for Services form for you to fill out. Once you have submitted the form, I will receive a notification and will contact you to set up an appointment.
