Catherine Courson is a licensed clinical social worker, providing telehealth counseling using a HIPPA compliant website. She is a preferred provider for many medical insurance and employee assistance companies.

SCHEDULING:
Sessions are by appointment only.

TO CONTACT ME:

1. Call me at 502-473-7028 and inform the answering service that you would like to schedule an appointment.  Calls are generally returned within 1-3 days.

2. You may message me, via the contact tab on this site.

3. When I return your call, we will cover the following:
•    Reason for seeking counseling.
•    Referral Source
•    Appointment setup and portal access

4.  Fee Schedule:
•    Initial first session fee is $125.00
•    Following sessions are billed at $100.00
•    If using medical insurance, please note that you are responsible 

        for fees not covered by insurance.  i.e. deductible, copays, 

        and/or co-insurance. The fees will be due at time of session.
•    If using Employee Assistance Program (EAP), you will be                          responsible for any sessions beyond those authorized.
•    The rate for a no show appointment is $75. 

*Please Note FMLA, Short and/or Long-Term paperwork is for established clients. You will need to have a minimum of 3 sessions prior to requesting the completion of the forms and also seen 2-3 times per month during the course of treatment. Insurance does not cover the cost of the forms.  Therefore, the rate is $75 per hour paid in advance.

Please Note:

You have the option of requesting 1 free copy of your session notes, per year. The notes will be delivered via this HIPPA compliant Telehealth portal. Please allow 2 business days for electronic delivery.

Treatments that are court ordered

Social Security Evaluation Forms

Service Animals Request Forms
