Our office will need to receive the forms back no later than 48 hrs prior to your appointment.  If your appointment was scheduled within a 48 hr window, please return immediately so we can ensure your appointment is confirmed.  

Please note, after reviewing the forms, if the provider feels the patient’s concerns are out of their scope of practice or if it is determined there is a conflict of interest, the provider will message the patient through the portal with alternate referral options.
